LONDON, Ont. -- The Special Investigations Unit (SIU) has released the identity of a 28-year-old woman who died after she was found in medical distress while in London police custody.

Amanda Bolt of London was reportedly arrested on the afternoon of Nov. 2 and taken to a cell where she was found unconscious the next morning, administered naloxone and taken to hospital.

She was pronounced dead in hospital 10 days later on Nov. 13.

After not initially becoming involved when the incident was reported by police the day she was hospitalized, the SIU invoked its mandate five days later after "learning further information."

Her brother Chris Bolt says the family is looking for answers, but will focus on the good memories.

"The family misses her...we love her...we hope she's peaceful now."

He says his younger sister had been struggling with addiction for the past 10 years, but she seemed to have found renewed purpose recently.

"She was using her negative experiences with life and turning it into positives to teach other people...I want to know what steps they took to ensure that this didn’t happen - if any."
